Licensing requirements

The Florida Agency for Health Care Administration (FAHCA) requires that upon initial application, change of ownership (CHOW) or renewal of licenses, each licensing application should be accompanied by a Proof of Financial Ability to Operate form. This form has to be prepared and signed by a Certified Public Accountant (CPA).
